  • Signed Mortgage Partnership Finance Program Disclosure (provided)
  • Signed servicing disclosure statement (provided)
  • Current pay stub reflecting year-to-date income
  • W-2's for the most recent two years or if you are self-employed, two years tax returns with all schedules and a year-to-date profit and loss statement
  • Most recent two months statements for all deposit accounts including savings, checking, investment, IRA's and 401K accounts
  • Legal description of property--provide either a title policy, copy of deed or plat of survey, if available
  • Real Estate tax bill (for refinances only)
  • Copy of homeowner's insurance declaration page or, if not available, please provide name and phone number of insurance agent
  • Monthly mortgage statement of current lender (for refinances only)
